Pre-Certifications
All inpatient services require precertification with ARC Administrators. You may complete the Inpatient Services Preauthorization Form and fax it along with the clinical information to the number on the form.
Some outpatient services require precertification (such as MRI, Therapy, etc.).
If you are requesting a precertification for an outpatient procedure you may complete the Outpatient Services Precertification Request form. Please include clinical information and fax to the number on the form.
